Document Submission for Online Profile Reports

If you represent a continuing care retirement community and would like to have a profile report for your organization featured in My LifeSite's database we only need a few items from you and we'll do the rest of the work. You'll have a chance to review your report for accuracy when it is completed.

What we need:

  • Most recent disclosure statement, including audited financial statements. (In states where disclosure statements are not required, please send a sample residency contract, audited financial statements, and a pricing page.)
  • An image file so we can include a picture of your community

There are two ways that you can submit your documents:

  1. Simply provide the document to us as an attachment to an email. You may send the email to us at info@mylifesite.net.
  2. If you prefer to send your documents via a secure online portal then proceed with the instructions below.

Step 1 - Please enter the name of your community and your email address. You will receive an email with an access code that will be required in step 2 below.


Step 2 - Once you receive your access code, please enter it below to access the document interface.
